Welcome Referees! The following list of what is required to submit to have a complete registration.

1. On-line Referee Registration electronically submitted through www.powersoccerusa.net (on this page)
   
   
2. On-line Code of Conduct per referee electronically submitted trough www.powersoccerusa.net
   
   
   • After submission of this page, you will be sent to another page to fill out the USPSA Code of Conduct. You must submit both forms before your registration will be accepted!
     
     
3. Waiver and Release of Liability and Publicity printable form (download here) must be submitted via postal mail (for insurance purposes) to the USPSA Secretary.
   
   
   • Waiver and Release of Liability and Publicity Forms should be completed and mailed to:

4. Proof of Referee Liability Insurance. If you are referee and cannot provide proof of liability insurance, you may purchase through the USPSA for $25 (download application here).
   
   
   • Checks should be made out to "USPSA" and mailed to:
